The 4-day Latent Cause Experience™– Where It All Begins

The Latent Cause Experience™ (LCE) is Failsafe’s foundational seminar. Everything else follows it.
​
LCE changes how people see failure. Instead of asking who caused the problem, attendees learn to ask what allowed it — and what it is about the way we are, and the way I am, that contributed. The course uses real events and walks the Latent Cause Analysis process step by step so the method is practiced, not just described.

Leaders leave better able to lead an investigation. Participants leave ready to take part in one. Managers leave knowing how to support the process without turning it back into blame.
​
The ideas do not stay in the classroom. People take them to work, home, and every problem that follows, large or small. The lasting takeaways are growth in leadership, learning, and an understanding of latency — the hidden conditions that keep the same failures coming back.

This is where the Latent Cause Mentality is installed. Without it, the 1-day rollout and the Principal Investigator Experience have nothing to build on.
1-day Latent Cause Analysis

The 1-day is how an organization that has already completed the 4-day Latent Cause Experience spreads the method to other sites and crews. It is not a replacement for the 4-day.
People who have been through the 4-day leave ready to lead. Everyone else still needs the same language. This day gives additional crews a working grasp of Latent Cause Analysis so they can run it on smaller incidents and serve as team members on larger ones.
Attendees leave with a practical way to investigate a problem and with a clearer view of latency as the true cause of recurring failure. Most important, they learn why these two questions matter:

The Principal Investigator Experience

This two-day course is for people who have completed the 4-day Latent Cause Experience and need practice leading high-consequence LCAs.
It is highly interactive. We review the LCA process, then walk through it step by step using years of field experience. A large part of the course is the stakeholder meeting — not the agenda on paper, but the small things that decide whether the meeting produces learning or collapses into blame.
That includes reading the room, noticing who is defending and who has gone quiet, keeping control without becoming the enforcer, and using evidence to drive the conversation so opinion does not take over. Participants practice when to slow the room down, when to bring people back to the facts, and how to keep the two questions in play: what is it about the way we are, and what is it about the way I am.
The course ends with how to report and communicate findings so the lessons actually get shared. Participants leave more ready to lead a serious investigation without defaulting to blame or skipping evidence.
